I am considered an out of network provider and therefore I do not accept any insurance. I am specifically unable to see patients who are currently under Medicaid and Medicare at this time.
At the time of your visit, I can produce a bill for you to submit to your insurance company for possible reimbursement.

Practice Agreement
-
Policies are covered in detail on the new patient intake forms. Completion of these forms is requested prior to your first visit with Dr. Robinson.

Fees
-
Payment is due at each visit. I accept cash, checks, debit card and credit cards.
-
I require each patient to have a valid credit card on file to charge for any outstanding balances.
-
Fees of $25 apply to completion of school medication forms completed between visits.
-
A fee of $50 is assessed for medication prior authorizations that require > 15 minutes of time. Fortunately, most medications do not require prior authorizations, or if they do they can be handled quickly and efficiently.
-
If more lengthy paperwork is required for any reason you should schedule an appointment and have it completed in session. Requests to complete paperwork or write letters outside of a session will incur fees for time spent at the rate of $400/hour.

Cancellations and Missed Appointments
-
Your appointment is reserved exclusively for you, so please give at least 48 business hours notice for any appointment which you are unable to keep. Full fees are charged for missed appointments and cancellations with less than 48 business hours notice. Please note that insurance does not reimburse any portion of these missed appointment fees.
-
Missing appointments interferes with the efficacy and continuity of your care. A pattern of repeatedly missing appointments is grounds for termination of care.
